Sutton Community Health Services offers a wide range of services for adults including community nursing, treatment and care for long-term conditions, podiatry and speech and language therapy

  • Community nursing services

    Our services which help and support those with disabilities and long-term conditions in their own homes.

  • End of life care

    The Community End-of-Life Care Team is a resource for GPs, Community Services and Care Homes. The team provides monthly support visits to nursing homes incorporating teaching sessions and clinical rounds.

  • Specialist therapies

    These services help patients who need support with speech and language, diet and nutrition and for those who have difficulty eating, drinking and swallowing.

  • Services for people aged 65 and over

    Our Older People's Assessment and Rehabilitation Service (OPARS) and Falls Prevention Service support patients during and after rehabilitation.

  • Prevention of Admission Team

    The Prevention of Admission Team offer a range of services to support patients in their homes, in order to prevent them having to be admitted into hospital.

  • Integrated Musculoskeletal Service

    The Integrated Musculoskeletal Service supports patients with problems of the body's joints, ligaments, muscles, nerves, tendons, and structures that support limbs, neck and back.

  • Neurotherapy services

    Our neurotherapy services support patients with neurological conditions such as Parkinson's or stroke.
