Rapid response

Our Rapid Response Service facilitates the prevention of unnecessary patient admission and is based at St Helier NHS Trust.

Overview of the service

The Rapid Response Team is a multidisciplinary health team made up of physiotherapists, occupational therapists, nurses and assistant practitioners. The team ensures patients receive the appropriate treatment by working closely with community services, social services and voluntary agencies. We reduce the risk of readmission with the use of follow-up home visits where appropriate.

Who can access this service?

This service is available for patients who are

  • Over the age of 18
  • Medically fit for discharge from hospital
  • Have an estimated length of stay in hospital of less than 48 hours (from admission to A&E)

  • Nurses
  • Occupational Therapists
  • Physiotherapists
  • Assistant Practitioners

Is patient self-referral available?

No, patients can only be referred from within the hospital.

Is transport provided?

  • No, transport is not provided by this service.

Are home visits available?

  • Yes. We visit patients' homes in order to assess their functional ability (how they cope with everyday tasks) within their home environment.
