Prevention of Admission Team

The Prevention of Admission Team supports unwell patients out of hospital by providing high quality assessment and medical support.

Overview of the service

The team of clinically experienced Nurses and Therapists provide a rapid assessment of a patient in their own home. The team work in an integrated way with health and social care partners to support the reduction of unnecessary and avoidable emergency admissions to hospital. The team  performs a comprehensive holistic assessment of all referred patients within 2-4 hours, making onward referrals to the appropriate community and social services, liaising with the referrer and patients GP of the outcome of the carried out assessment. 

Intermediate Care Team

The team is designed to support both early discharges from hospital to return patients back into their own home and to support patients in their own home in order to prevent an avoidable hospital admission.  They do this by assessing a patient’s level of function regarding self-care and planning a rehabilitation programme jointly with the patient.  The programme of rehabilitation is provided by members of the team which consist of Physiotherapist, Occupational Therapists and Rehabilitation Assistants. The service is delivered both in the patient's own home or in one of the intermediate care beds in Sutton through joint partnership working with identified local care homes.

This service is not suitable for patients who have:

  • An acute or unstable mental health diagnosis 
  • Identified as requiring social care intervention only. 
  • For Persons under the age of 18 years
  • For persons not registered with a Sutton GP

Hours of Service

This service operates a seven day service over 365 days a year. Operating times are 8am until 7pm.
