End-of-life care

The Community End-of-Life Care Team is a resource for GPs, Community Services and Care Homes. The team provides monthly support visits to nursing homes incorporating teaching sessions and clinical rounds.

Overview of the service

The aim of the End-of-Life Care service is to support nursing homes' care for patients who are approaching the end of their lives. The service provides advance care planning, symptom control and bereavement support. We also advise and support GPs with their end-of-life care patients. 

The service provides teaching sessions for community services and the team attend local GPs end-of-life care/Gold Standards Framework (GSF) meetings to advise on best practice.

Who can access this service?

All adult residents in nursing care homes in Sutton can access the End-of-Life Care service.

Macmillan Nurse Consultant and End-of-Life Care Nurse Specialists (CNS)

Is patient self-referral available?

No, patients need to be referred by their GP or care home.

Are home visits available?
